In order to participate in the iGEM 2012 High School competition, all student teams must obtain consent from the principal at their school. Team leaders may download the consent form below. The team registration will not be approved until:

1. The team leader obtains and submits to iGEM HQ principal consent AND
2. All team members are listed on the official team roster AND parental consent forms are received by iGEM HQ for each team member.
