Consent Forms/Parent

In order to participate in the iGEM 2012 High School competition, all student team members must obtain parental consent. Team leaders may download the consent form below. The team registration will not be approved until:

  1. All team members are listed on the official team roster AND
  2. Both participant/parental consent forms AND principal consent forms are received by Headquarters. See the principal consent form page for details.
